Abstract
Rheumaticheartdisease(RHD)isthemostimportantlong-term
consequenceofacuterheumaticthatnowlargelyaffectstheworld’spoorest
populations.Morethan30millionpeopleworldwidehaveRHDandmore
than300,000deathsand10milliondisability-adjustedlife-yearsoccurper
yearasaresult.Althoughdiseaserateshavedeclinedglobally,RHDis
increasinglyconcentratedamongthepoorestandmarginalized.Heartvalve
involvementinRHDcanbedetectedandgradedclinically.However,
accuratediagnosisrequiresechocardiography.Standardizedcriteria
developedbytheWorldHeartFederationallowsforunambiguousdiagnosis
ofRHDandareespeciallyusefulintheabsenceofahistoryofRHD.Much
ofthedamagefromRHDislimitedtomitral,aortic,andtricuspidvalves,
andseveredamagerequiresexpertcardiovascularcareintheformof
correctivesurgery.Interventionaltherapywithpercutaneousmitralballoon
valvotomyislargelylimitedtoselectedpatientswithpuremitralstenosis.
Thischapterseekstoprovideacontemporaryandcomprehensiveupdateon
rheumaticheartdisease.
