Conclusions: Our study showed that half of the cases admitted due to unconjugated hyperbilirubinemia at
the neonatal department of Pediatric Hospital N02 were severe or critical hyperbilirubinemia, and the clinical
estimation of jaundice degrees at admission was much lower than the one implied from the measured bilirubin
level. This result recommends to equip transcutaneous bilirubinometry device, at least at outpatient setting of
pediatric hospitals, in order to enhance the accuracy of primary evaluation of bilirubin levels, permitting the
prompt detection and follow-up of cases of severe hyperbilirubinemia.

ĐẶT VẤN ĐỀ
Đối tượng nghiên cứu
Vàng da (VD) tăng bilirubin gián tiếp xuất
hiện ở hầu hết trẻ sơ sinh. Đại đa số trường hợp
VD là lành tính, nhưng vì độc tính tiềm tàng của
bilirubin trên não, cần theo dõi sát để phát hiện
và cho nhập viện điều trị kịp thời những trẻ có
thể bị tăng bilirubin/máu (BM) nặng, nhằm dự
phòng bệnh lý não cấp do bilirubin và VD nhân.
Điều trị chọn lựa là ánh sáng liệu pháp (ASLP),
thay máu chỉ được thực hiện trong những
trường hợp quá chỉ định hay thất bại của ASLP.
Trẻ nhập viện vì VD tăng bilirubin gián tiếp
tại khoa Sơ sinh Bệnh viện Nhi Đồng II trong 3
năm 2009-2011.
