introducedwhatisperhapsthemostdisruptivesurgicalinnovationin
congenitalcardiacsurgeryaprocedurethatsimultaneouslyeliminatedvolume
loadandcyanosisamongpatientswithafunctionallyuniventricularheart.These
operationswereinitiallyappliedtopatientswithrightheartobstructivelesions


(tricuspidatresiainparticular)andweretermedrightheartbypassprocedures.
Thesuccessofthesepioneeringoperationshasbeenfollowedbymultiple
surgicalmodificationsoftheoriginalconcept.Withalong-termpalliativeoption
nowavailableforthepatientwithafunctionallyuniventricularheart,additional
ingeniousandequallydisruptiveoperationswerecreated,includingthe
Norwoodandhybridprocedures,thesuperiorcavopulmonaryconnection(the
bidirectionalGlenn),andothers.Thesenewerprocedureshavecontinuedto
undergomodificationstobetterallowsurvivalininfancyandminimizerisk
factorsfortheeventualmodifiedFontan(orperhapsmorecorrectlyFontanKreutzer)procedure.Incombination,thesecomplexsurgicalprocedurescanbe
consideredstagedreconstruction.Alongwithmedicalmanagementand
monitoring,theyhavecometobeknowncolloquiallyastheFontanpathway.

describedmorphologically,followedbyadescriptionofthephysiology,surgical
