Abstract
Thearterialductisafetalbloodvesselthatisprogrammedtocloseshortly
afterbirth.Ifthevesselremainspatentbeyond3monthsoflife(a
persistentlypatentarterialduct)itcanresultinvolumeloadingoftheleft
heartandshouldbeclosedeithersurgicallyorbyacatheter-based
procedure.Apatentarterialductiscommoninthenewborn,particularly
prematureinfants,andcancontributetopersistentrespiratoryproblems.
Earlysymptomsareuncommonbutmayincludedyspneaandfailureto
thriveinthemostextremecases.Anuncorrectedpatentductmayleadto
congestiveheartfailurewithincreasingage.Insomecongenitalheart
lesions(e.g.,atrioventricularconcordance,ventriculoatrialdiscordance)a
patentductistheonlypathwaywhereoxygenatedbloodcanmixwith
deoxygenatedblood.Insuchcases,prostaglandinsareinfusedtokeepthe
ductopen.Nonsteroidalantiinflammatorydrugscanbeusedtoclosethe
ductintheprematureinfantbutshouldnotbeadministeredtothosewith
congenitalheartdisease,inwhomtheductisusedtoaugmentpulmonary
bloodflowormaintainsystemicflowinleftheartobstructivelesions.This
chapterdetailswhatcontrolsductalpatency,itspathophysiology,andits
effectonthecirculationinheathanddisease.
