2020 Community Scholarship Application
Dear Applicant:
The Cincinnati Queen City Alumnae Chapter of Delta Sigma Theta Sorority, Inc. will distribute scholarships to
African-American females in the Greater Cincinnati Metropolitan Area that are currently high school seniors and
plan to attend a college or university upon graduation. Recipients of the scholarship will be based on academic
excellence, community service involvement, financial need, and a short interview, if requested. Interested
applicants must have a minimum grade point average of 2.5 on a 4.0 scale.

Delta Sigma Theta Sorority, Inc. is an international sisterhood committed to public service. It was
founded in 1913 by 22 undergraduate women at Howard University and has grown into a sisterhood of
over 200,000 African-American college-educated women and has over 900 chapters in the United States
and abroad. Delta Sigma Theta Sorority, Inc. nationally provides an array of public services through its
Five Point Programmatic Thrust: Economic Development, Educational Development, Physical & Mental
Health, Political Awareness & Involvement, and International Awareness & Involvement.

Cincinnati Queen City Alumnae Chapter (CQCA)

Chartered in 1989, CQCA sponsors several programs based upon the national Programmatic Thrust
including Delta GEMS and the Dr. Betty Shabazz Delta Academy. CQCA has also sponsored several local
events including voter empowerment, domestic violence awareness, and visits by nationally acclaimed
artists to our city. CQCA has awarded over $200,000 in college scholarships to young men and women
throughout Greater Cincinnati.

Below is a checklist of all the items that must be submitted as part of the application packet by the deadline of
FRIDAY, MARCH 27, 2020. (Use this checklist as your guide for submitting your
application).
All items must be mailed to the sorority post office box and RECEIVED on or before FRIDAY, MARCH 27,

Typewritten essay
Official transcript (with school seal or watermark) in a sealed envelope that
includes first semester grades of the current academic year. This transcript
should include your unweighted GPA on a 4.0 scale.
Two current letters of recommendation: 1) One from a school staff person (i.e.
school counselor, teacher, etc.) regarding qualities such as academic excellence,
leadership, initiative, etc. 2) One from a community-based person regarding your
participation in a community service activity or organization.
Letters of recommendation should be written by persons who are not related to the applicant. Letters of recommendation should be dated
on or after January 1, 2020. The letter verifying community service must come from an official of the organization that was the recipient of
services rendered. The letter verifying community service must be submitted on official letterhead. Please have the letter speak to the
number of hours and type of community service activity you were/are involved in. Community service may include services rendered as a
member of an organization (i.e. Jack and Jill of America, Inc., Xinos Club, church youth groups, school organizations, etc.), or individual
volunteerism (i.e. hospital volunteer, tutor, participation in charitable activities, etc.). The letter from a school staff person must come from a
current employee of the school. To ensure recommendations meet our guidelines, we strongly encourage you to give the attached Guidelines
for Making Recommendations document to anyone submitting a letter for you. Individuals whose mother is a financial member of the
Cincinnati Queen City Alumnae Chapter are not eligible for this scholarship.

Any application materials received after the deadline date of FRIDAY, MARCH 27, 2020 will not be accepted. Confirmation will be sent to
applicants acknowledging receipt of the application packet. Selected applicants will be contacted to schedule an interview that they must
participate in order to be eligible for the scholarship.

Applicants who qualify will be contacted by the scholarship committee and informed of the interview date, time and
location.
The information on this application is true to the best of my knowledge. I understand this application is for a scholarship (which is

Guidelines for Making Recommendations
Applicant: Please share these guidelines with the people writing your letters of recommendation.
GENERAL GUIDELINES FOR LETTERS OF RECOMMENDATION
Δ Persons who are related to the applicant should not write letters of recommendation.
Δ The letters of recommendation listed below cannot be combined into one letter.
Δ The letters must be dated on or after January 1, 2019. The letters must contain the appropriate
signature and date.
Community-Based Person Regarding Participation in a Community Service Activity or Organization
The written summary should include the following:
Δ Name of the organization
Δ Name and position of the person providing the information
Δ Length of time the applicant has been involved with the organization
Δ Explanation of the applicant’s involvement with the organization
Δ Identification of current and past responsibilities of the applicant
Δ List of current and past roles and/or responsibilities of the applicant
Δ Outline of the applicant’s accomplishments in the organization
Note: The letter of recommendation from a community-based person must come on official letterhead and

must be written by an official of the organization that was the recipient of services rendered.

School Staff Person (e.g. Counselor, Teacher)
The written summary should include the following:
Δ Name and position of the person providing the recommendation
Δ Length of time and in what capacity the person has known the applicant
Δ State any public service activities the applicant is or has been involved with, of which you are
aware
Δ What personal characteristics or qualities does the applicant possess which may be an asset in
determining if he/she should be awarded this scholarship (e.g. academic excellence, leadership,
initiative)?
Note: The school staff person providing the recommendation must be a current employee of the school.

Important Notice to Individuals Writing Letters of Recommendation
Any letters of recommendation submitted on the applicant’s behalf may be shared
with the applicant and/or his parent(s)/guardian(s) for personal development
purposes. If a letter of recommendation cannot be shared freely then it is not
recommended that a letter of recommendation be provided on the applicant’s
behalf.
