1.

Preface
The safety is of paramount importance at all times and consideration of such
safety shall take precedence over any and all other consideration. The
contractor shall throughout the execution and completion of the wok, having
full regard for safety of all persons entitled to be upon the site and keep the
site and the work in an orderly state appropriate to the avoidance of danger
to such person.
The primary objective of the Safety Plan is to avoid human distress and
financial loss during the performance of all work on field and to ensure
effective control of them. It is also expected that every employee does his
best effort to create a safe environment to work. The Safety Program
outlines the procedures essential for the protection of men and materials in
work.

Every staff that supervises or directs the work of craftsmen shall pursue his
best efforts to assure the safety of each employee under his supervision or
direction. In addition, he shall be accountable for the protection of
equipment and property within his area of responsibility.
Within 30 days of the award of the Contract, the Contractor shall submit to
the Project Manager for his approval, a complete safety program. The
Program shall contain the following as a minimum:

Administrative responsibilities for effecting the Accident
Prevention

Plan

(identification

and

accountability

of

Contractor personnel responsible for accident prevention).



Local requirements, if any, which must be compiled with, i.e.,
noise control, traffic regulation, etc.



The method the Contractor proposes to control and coordinate work of his Subcontractors.



Safety Plans for layout of temporary construction buildings
and facilities, including how the Contractor plans to control
those of his Sub-contractors (include sanitary facilities).

Plans for initial and continuing safety education, and training
for the Contractor's employees.




Plans for traffic control and marking of hazards to cover haul
roads, highway intersections, utilities, bridges, restricted
areas, etc.



Methods for pedestrian road closures and alternate routes,
foot bridges and barricades; directional signs and warnings
and pedestrian traffic control. This plan shall be co-ordinated
with appropriate local People's Committee officials.



Plans for maintaining continued job clean up and safe access.



Plans for fire protection and dealing with emergencies
(ambulance service, fires, first aid, medical evacuation).



Plans for inspection of the job sites by competent persons
including reports to be kept, results of the inspection, and
corrective actions taken. Particular attention being paid to the
utilisation of a "Safety Violation Report Form".

(ii)




Procedures to be used for accident investigation.



Description of safe clearance procedures.



Description of office trailer anchoring system.



Contingency plans for severe weather.

Activity Hazard Analysis Section


An Activity Hazard Analysis shall be developed for each
contract activity and operation occurring in each major phase
of work.



The Contractor shall develop the plan to identify the sequence
of work, the specific hazards anticipated, and the control
measures to be implemented to minimise or eliminate each
hazard. The Activity Hazard Analysis shall be job specific and
shall address the following major points:




Activity being performed (identify major phase), to include
but not be limited to the following :

earth moving (equipment operations, backing signals, etc.)



trenching operation (shoring)



lifting equipment operations (crane certification, rigging, etc.)



welding and cutting operations (P.P.E.)



blasting operations (regulations)

Hazards to be controlled in each activity, and means of

The Plan will be reviewed by the Project Manager and a
meeting held within two weeks of submittal to discuss and

approve the plan.

2.2.2 Safety manager



Safety manager is a competent and experienced person who is suitable for
investigation and checking the Safety Plan obedient, and especially in
detail but not restrictedly check the Safety Plan Activities in a rolling-up
program that depending on different time will be submitted to Consultants

for approval.



The contractor will make no action on site until the Safety manager does
unless Consultants release an official approval document



The contractor must not command the Safety manager to go quit site
unless there is permission in word from Consultants. Within 14 days from

the notice of transfer of that employee, the contractor will choose an alter
Safety manager and consultants will give final decision.


The contractor authorizes the Safety manager and officers to indicate the
contractor’s or sub-contractor’s employees to end all of their work and
carry out the urgent and reasonable actions to ensure the site safety and
prevent unsafe actions or violations to Safety plan or the Law




Evaluate the needs and propose for safety equipments, first-aid and fire
prevention.

Make

sure

that

the

safety

equipments

are

in

good

maintenance conditions.


Making and maintaining a daily site diary, this recording all relevant
matters concerning site safety, inspections and audits, related incidents
and the like. The site diary shall be available at all times for inspection by
the Consultants.




Safety manager reports to the project manager and informs the decision
on Safety control plan to workers.



Safety manager makes workers list and indicate the leader of workers’
groups and implements the workers labour safety training plan

2.2.3 Vice Safety manager
Vice Safety manager is indicated by CEO under the approval from
Consultants and will offer help manage, labour safety monitoring during
construction time. In case Safety Manager does not present on site, Vice
Safety Manager will alter, monitor and decide on matters relating to labor
safety. In case serious problem happens, Vice Safety Manager reports to
Project manager, Safety Manager can solve the problem.
Project manager will indicated a new employee being in charge of site nurse,
carrying out the training of handling health problems for workers/officers on
site.
Site nurse must have knowledge of medical, health safety, first-aid and
problems handling.
Site nurse must has to present on site regularly to monitor activities and
make first-aid and solve medical problem shortly if any.

The jobs and actions to do of Vice Safety Manager – Site nurses are as
follows:


Ensure workers health



Monitor the safety training in accordance with site’s requirements



Cooperate in first-aids on site, ensure that employee is basically equipped
in case the nurse is absent.



Investigate all accidents, injuries, fire, and financial loss and other
incidents relating to safety, meanwhile prepare/submit necessary reports.



Site nurse has to distinguish between career accidents and time-wasting
and injury-causing incidents, monitor to control the situations and fill the
form of suitable accidents report.




Making and maintaining a daily site diary, this recording all relevant
matters concerning site safety, inspections and audits, related incidents
and the like. The site diary shall be available at all times for inspection by
the Consultants.
2.2.4 Site Engineer
The Construction manager is directly responsible for the control of site
activities of this Project. They are the key personnel in the implementation
and maintenance of an effective job site health and safety program,
assisting the Safety Manager
The standards for a good safety programme are established by the Safety
Manager and Construction Manager. The Site Engineers will be directly
responsible for the success of Safety Control Plan as they are in direct
contact with the Sub-Contractors. Their abilities to instill good health and
attitudes to the site safety.
Sub-Contractors will reflect the success of the Safety Control Plane on site.
Site Engineers are responsible for a wide range of activities. They must plan
their health and safety activities with care and good effort as they have to
carry out other portions of their work programme such as cost, manpower
allocation and scheduling of the respective work within this Project.
2.2.5 Safety checker team

Safety checker team is established under the decision of Site engineer which
was approved by the safety manager. Safety checker team is trained with
labour safety and situations handling.
The specific responsibilities of the safety checker team are as follows:
1.

Review all work and all new personnel for safety needs. Complete
familiarization with safety requirements is an integral part of the total
job responsibility.

2.

Continual monitoring of the safety attitudes of Sub-Contractors and their
responsibilities towards the safety of their trade and participations in the
job safety effort.

3.

Enforce on compliance with all established health and safety regulations
before and during performance of the work.

4.

Conduct health and safety inspections and make corrections on a daily
basis.

5.


Initiate plans prior to the start of work to supply the tools, materials, and
equipment necessary to do the job safely.

6.

Assist with accident investigations to ensure proper reporting and
documentation with prompt corrections made after accident and to
prevent any recurrences.

7.

Co-ordinate

planning

and

work

activities

with

other

supervisory

personnel performing work within, or adjacent to, their assigned work
areas.
2.2.6 Sub- contractor’s responsibilities

1.

To ensure that all the respective agents and employees conform with
the contents of this Safety control plan.

2.

To supply employees with appropriate safety equipment and appliances
when working.

3.

To attend Contractor’s safety meetings, when called upon, and to
conduct safety meetings for all his employees.

4.

To rectify unsafe conditions and hazards when required by contractor’s
Safety Manager and/or his staff.

5.


To maintain good housekeeping.

6.

Report all accidents to the Contractor’s Safety Manager or his staff
immediately.

7.

To seek first aid for all injuries, however slight, at the first aid station.

8.

To be entirely responsible for the implementation of safety measures
and full compliance with regulations of his company in respect of both
himself and all his employees.

9.

To ensure that all staff and worker engaged on the project are in sound
health and capable of carrying out their duties.

2.2.7 Operators / Workers
Operators or workers working with tools, plant, equipment, etc. are
responsible for their health and safety with others working around them.
Therefore, there should be sufficient instructions provided to them. They
shall be responsible for the followings:

Use proper, correct and well-maintained tools and equipment for the

jobs

2.

Wearing of efficient safety helmets and where necessary. eye, goggles,
ear

protection,

safety

harnesses

and

other

personal

protection

equipment for all personnel
3.

Maintain and keep tools in good condition at all times

4.

Report defects in plant or equipment or any potential risk to health and
safety to Supervisor

Avoid improvising work which entails unnecessary risk

2.2.8

Sub-contractor management

Contractor will provide sub-contractor with the copied versions of Safety plan
and bring them into all the clauses of subcontract documents to ensure the
plan obedient by all levels in subcontract.
Unless there is official approval document from Consultants, contractor will
require all subcontractors to authorize a representative in charge of safety to

present on site during the implementation time of the contract related to
subcontractors. When Consultants approve, Safety manager or safety staff,
making to affection to other duties and responsibilities, shall ensure to some
practical extent that all staff provided by subcontractor have knowledge
about suitable issues of Safety plan and law regulations.
3.

Safety Instruction and Training
The Safety Education shall be enforced for workers throughout the daily
activities, while it raises safety consciousness of workers to prevent accident.
a) Orientation for the Newcomers.
Newcomer shall have an orientation directed by the Safety Engineer or group
leader to be familiar with working atmosphere.
b) Education for a New Activity/Method.
A group leader shall give a description of an activity with tips of safety maters
to eliminate any of unsafe factors.
3.1 Instruction and Training on the Job
1.

Contractor shall hold a Training course on first-safety. All workers shall
attend this course in the first week of mobilization to site. Safety

manager and vice safety manager shall edit the training course book, be
in charge of training and instructing workers in the course.
2.

Training periods shall ensure the effectiveness, provide workers with
information about safety regulations, safety equipments use, and
situations handling in incidents. At the end of the course there shall be
an comprehension test to check the understanding of workers. Those
who fail the test shall not be allowed to take part in the construction.

3.

Contractor also hold periodic safety training course every six months.
Contractor shall ensure that all subcontractors’ staff attending in the
course which is appropriate to the job’s features, levels and time
according to subcontract. This regular course is for all workers on site
and supervisors as well as managers.

4.

Safety meeting: Safety meetings scheduled on regular basis shall be
held at least once a month with the attendance of safety manager and
safety representatives of subcontractors. Safety meetings shall be
informed in advance to Consultants to decide the attendant. The
meeting minutes shall be made and sent to Consultants within 7 days
from meeting day.

Everyone required entering confined or enclosed spaces or atmospheres
with immediate danger to life, shall be instructed as to the hazards
involved and precautionary measures to be taken. Training by qualified
person on the use and care of such emergency and protective
equipment shall be conducted. Contractor’s staff shall comply with all
specific regulations in dangerous or potentially dangerous areas

6.

Install the safety signal system on site. They shall be in accordance with
regulation and have suitable size to the installation positions. Especially,
contractor shall pay attention to installing safety signal posts at
dangerous areas such as electricity source, poison, flammable matters.

7.

All employees who have to handle poisons, caustic or other harmful
substances, are required to receive instruction regarding the safe
handling and use. Information and knowledge on the potential hazards,
personal hygiene, protective equipment and other personal protective
measures involved or required

8.

Personnel who may be required to use the rescue or life saving
equipment must be familiar with the locations of the equipment and
knowledge with training on using them .

9.

Personnel required to provide or receive signals, must be familiar with
the proper use of the signal system.

10. Personnel required handling or using flammable liquids, gases or toxic
materials must be trained in the handling, storage and use of them with
specific protection requirements
11. Whenever the employees are likely to expose to extreme temperature or
exceed atmospheric pressure, instructions on recognition, potential
hazards, avoidance of injury and first aid procedures to be applied for
any incident
12. Contractor shall provide training on the handling of emergency
situations that may arise from the use of equipment in the Project.
13. Instruction to use and operate any heavy equipment will be given and
personnel required to use these equipment shall demonstrate their
proficiency in the operation process.
14. Any personnel required working with energized power and transmission
lines shall be instructed and trained in the live line bare-hand technique
and safety requirements before allowed to use.

3.3 Special safety training

3.4 Emergency communication
1.

The emergency communication system shall be established and posted
at the convenient access-ways, in all site offices, in all security towers /
gates, medical center, in order for speedy rescue of the accident victims,

timely communication with local fire brigade office, prompt response
from first aid staff, and the soonest possible evacuation of the workforce
if necessary in a serious emergency

2.

First Aid station shall be manned during all working hours. This will be 24
hours if necessary. First aid center will be equipped with telephone and 2
way radio.

3.

Medical Staff will be well informed as to the project layout and be taken
on regular site visits as the project changes so that they always know
the access routes and stairs to ensure quickest access to injured
persons.

4. Implementation of the Daily Work Meeting and Weekly Progress
Meeting
Daily Work Meeting shall be held once a day with attendance of all staff
and work group leaders. Agenda shall include:

a) Schedule
b) Arrangement among activities.
c) Procurement of material
d) Instruction for activities
e) Manner of activities
f) Fireproofing
g) Exclusion of unsafe action by workers.
An instruction sheet shall be issued in accordance with the decision and
confirmation of the responsible persons. In addition, Weekly Progress Meeting
shall be also held to monitor and update workforces, materials, and safety
matters.
5.

Recording and reporting of safety and reporting of Accidents
5.1 Recording and reporting of safety

The contractor shall maintain such records and make such report concerning
safety. Such report shall be submitted to Employer representative and the
engineer representative on monthly

The Contractor shall submit a summary report as part of the Monthly Progress
Report.

Prior to submission, the Contractor’s Representative or Project

Manager shall endorse the Report. Site safety reports shall address all
relevant aspects of site safety and industrial health regulations and, in
particular, report on all site safety audits undertaken during the period
covered by the report.

The contractor shall report to the Engineer details of any accident as soon as
possible after its occurrence. In the case of any fatality or serious accident the
contractor shall, in addition, notify the Engineer immediately by the quickest
available means

6.1 Personal Protective Apparel
Personal Protective gears like protective footwear such as safety boots,
rubber boots, gloves shall be worn by employees engaged in works that
require such protection. All points of entry for hard hat (safety helmet) must
be identified with the sign. Visitors to site must also adhere to the safety
requirements at the site.
1.

Protection to eyes and face for work that may cause injury due to
physical, chemical or radiation agents.

2.

Reflective or high visibility material should be worn for staff exposed to
vehicular traffic e.g. signalmen, spotters, inspectors, servicemen, etc.

3.

Protective nets for overhead protection shall be provided wherever the
public or workers are subjected to injury due to falling objects.

4.

Work near protruded vertical reinforcing steel shall not be permitted
unless provision to eliminate possible hazards is carried out.

5.


Protection to ears with ear plugs or ear muffs are required for sound
transmitted at working area that exceeds 85 dB.

6.

Insulating personal safety gears such as insulating gloves, insulating line
hose, etc. to be provided for electrical staff.

6.3 Site signal posts
Contractor shall install the signal posts on sit according to regulations. The
size of signal posts is appropriate with the installation positions so that people
can easily recognize and clearly understand the mentioned contents.
Especially at dangerous positions such as electricity sources, flammable
chemicals… the signal posts shall be installed and regularly monitored.
The posts to be installed on site shall be in accordance with law’s regulations.
The following picture demonstrates some signal posts that shall be used on
site.

Safety nets
Provide and install and maintain for the duration of construction all
required net for employees working in height. Nets shall be hung with
sufficient clearance to prevent contact with the surfaces or structures
below.

It must be emphasized that safety nets are to be used in conjunction
with handrails /barricades. They are not the primary means of
preventing workers and or objects from falling. Kick-boards on
scaffolding, safety harnesses, guard-rails etc., are primary means of
preventing falling items.

Where traffic or workers are permitted to be under a work area, safety
nets may be utilized to satisfy the overhead protection requirement.

Nets shall extend 2.4m beyond the edge of the work surfaces where
workers are exposed.

e)

The maximum mesh size of nets shall be 10 cm x 10 cm. The preferred
size is steel mesh or grill no bigger spacing than 5 cm x 5 cm.
