SUMMARY
EVALUATION OF EARLY RESULTS OF SURGERY THERAPY IN CHRONIC
OCCLUSIVE ARTERIAL DISEASE OF LOWER EXTREMITY
Subjects and methods: From 05/2009 to 04/2011, prospective study at Department of
Thoracic and Cardiovascular Surgery of Hue Centre hospital, we have performed 108 patients in
63 males and 45 females from 33 to 91 ages, who were treated chronic occlusive arterial disease
of lower extremity by surgery therapy.
Objectives: Evaluate the clinical symptoms anf the early results postoperatively.
Results: Majority of patients suffered from pains at rest or ischemic ulcers and necrosis, the
stage of disease was stage IV (55,56%) according to Leriche Fontaine. Obstructive lesions
mainly located in 3 shank arteries and 50% in both of legs, the highest rate was anterial tibial
artery with 81,48%, posterial tibial artery with 72,22%, and calf artery with 62,96%. All patients
underwent Echo Doppler two lower extremity arteries, in those 72 cases were performed contract
angiography to evaluate preoperative, 01 cases explorative operation. We have performes
72,22% sympathetic section, 16,67% bypass surgery, 11,11% amputation. During postoperative
following up, the rate of good response after sympathetic section was 79,48%, 77,78% in bypass
surgery, 22,22% obstructive after performing bypass, and then amputation.
Conclusion: The major stage when patients come to hospital was stage IV (ulcer, necrosis).
Almost cases were narrow and obstructive in 3 shank arteries. The indication of sympathetic
section was still high. Result of early surgery therapy by sympathetic section and bypass has
shown a good effect relatively.
