LABOUR ACCIDENT PROCESS

Objectives:
o Applicable to all HVB employees and 3rd parties after incidents, accidents

o Business continuity operation
o Good company and brand images
Responsibilities:
o Brewery Manager is responsible to report all fatalities, accidents, incidents.
o On- site employees are responsible for providing exact information relating to the
accidents
o On-duty Nurses are responsible for doing first aids properly and recording the accidents
occurred during their duty precisely (For making Minute of Labor accident investigation
within 24 - 48 hours as State form)
o Safety Manager is responsible for full investigation and fulfilling all documents required
by Law and Heineken timely and taking corrective and preventive actions quickly
o The investigation of an accident/incident shall start within 24 hours and shall be finished
within 2 weeks. The accident shall be analysed based on the 5-Whys methodology.
o Medical Manager is responsible for taking care of victims until recovery
o HVB Clinic will carry out the mandatory procedures as required by Law for employees
after stability of injured employees (Health and Disability Evaluation, Disability
compensation, Job transfer / arrangement), if any
